I understand and acknowledge that participation in any dance class, workshop, rehearsal or performance may result in physical injury. I assume all risks and responsibility for any injury or accident, which might occur to me or my ward during any of Shehnaaz Dance Academy LLC’s classes, rehearsals, performances, workshops, pop-up classes and/or activities.

In case of pre-existing physical, neurological or mental health conditions, I shall consult my physician before participation and the onus of avoiding, refraining from or discontinuing any movement during a class is solely my responsibilty. Shehnaaz Dance Academy LLC and its owners, heirs, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students are not liable or obligated to conduct/ curate/ advise about classes or routines suitable to my specific needs and condition. I also exempt, release, and indemnify Shehnaaz Dance Academy LLC and its owners, heirs, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Shehnaaz Dance Academy LLC.

I further hereby voluntarily agree to waive my rights and that of my heirs to hold Shehnaaz Dance Academy LLC, its owners, heirs, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand and acknowledge that, this activity may involve a test of a person’s physical and mental limits and may carry a potential risk ofserious injury, loss of personal property and death.

I exempt, release, and indemnify the following from any and all liability claims, demands or cause of action whatsoever from any damage, loss, injury, or death to me, my ward, or property which may arise out of or in connection with participation in any classes or activities conducted by Shehnaaz Dance Academy LLC.

  • Newport Swim and Fitness, JC (33 River Drive South, Jersey City)

  • Boys and Girls Club, Jersey City (225 Morris Blvd, Jersey City)

  • Ripley Grier Studios (305 West 38th St, New York City)

  • Bhakti Barn Yoga, Millburn (391 Millburn Ave, Millburn)

  • Edgewater Community Center (1167 River Road, Edgewater)

This agreement is entered into without any duress or coercion and is not to be interpreted as an agreement between two parties of equal bargaining strength. Both the participant and Shehnaaz Jagasia, doing business as Shehnaaz Dance Academy LLC, agree that this agreement is clear and unambiguous as to its terms, and that no other evidence will be used or admitted to alter or explain the terms of this Agreement, but that it will be interpreted based on the language in accordance with the purposes for which it is entered into. If I am signing this waiver for a minor(s), I certify that I am the parent or legal guardian and have the right to waive these rights.

By signing this agreement, I accept that I voluntarily agree to all the above terms and conditions as a participant or as a parent/ legal guardian of the minor participant.
